An exploding oxygen cylinder was the probable cause of a hole in a Qantas jet which was forced into an emergency landing, a safety official confirmed.
Julian Walsh, of the Australian Transport Safety Bureau, said it appeared the tank had exploded through the passenger cabin floor.

"The ATSB can confirm that it appears that part of an oxygen cylinder and valve entered the passenger cabin and impacted" a door handle, Mr Walsh told reporters.
